รูปถ่ายหน้าตรงขนาด 2 นิ้ว 2 รูป
แบบฟอร์มคำร้อง (รับได้ที่สถานทูต)
ในบางกรณี เจ้าหน้าที่อาจขอข้อมูลบุคคลอ้างอิงในประเทศไทย เช่น เบอร์โทรญาติ หรือที่อยู่ตามทะเบียนบ้าน เพื่อยืนยันตัวตน
